You will need Adobe Acrobat Reader to open the form, and once you have filled in the boxes, you can print the form to submit to your dean, chair and lead. Detailed instructions can be found here.
As the instructions state, you cannot save the completed form unless you have the full Adobe Acrobat program. If you would like a record of your submission, and have access to one of the newer networked Xerox copiers, you can email the form to yourself along with dean, lead and chair.
